Important Update #13: Embryo freezing

Freezing of eggs, sperms, zygotes, embryos and blastocysts  was being done since many years by slow freezing method. However this was not very efficient for eggs and blastocysts. With the  development of Vtrification a revolutionary technique of freezing, very high success rates have been achieved and post thaw survival, and pregnancy rates are equal to fresh. This has opened the gates to successful egg and embryo banking. It has also made IVF safer, more patient friendly. Pregnancy after frozen transfer is less complicated and babies are healthier than fresh transfer. Implantation rates are better in a natural cycle as done for frozen embryos than a stimulated cycle. So all Ivf couples insist on freezing your extra blastocysts!

Important Update #12: Blastocyst transfer gives better pregnancy rates than day 3 embryo transfer.

Embryos are cultured in the IVF lab for 3 to 5 days. Previously all IVF centers in the world transferred embryos on day 3 when the embryo has 8 cells. When cultured till day 5 fewer embryos become blastocysts, and these are the ones which have higher implantation potential. Implantation rates of day 3 embryos is 30 percent while for day 5 blastocysts implantation rate is 50%.o n day 5 lesser embryos are transferred, so chances of high order multiple pregnancies is reduced. So all going in for an IVF cycle ,go for a blastocyst transfer!

Important Update #9 : Couples carrying single gene disorders

Couples carrying single gene disorders, can have a healthy child with their own DNA, and just the affected gene modified or removed! A new technology has come in the IVF laboratories called CRISPR Cas9genome editing technology, where one can cut an affected gene and paste a new one. This can eradicate single gene disorders from the world! It can also start a new era of designer babies, like more intelligent, good looking, tall, slim etc. Although a very easy technique, can be done by any lab, scientific community is putting a lot of restrictions on human germline editing, restricting it only to dominant single gene disorders, where the affected child can have a good quality of life. Germ line editing for social reasons like looks and intellect is not allowed in humans.!

Important Update #7 : Azoospermic men can have their own genetic baby!

Azoospermia is a condition where the semen has no sperms. Sperm are produced in the testis and stored in a highly coiled tube near the testis, called epididimis, before they are ejaculated.very often due to infection, congenital defects, vasectomy surgery, the way for sperms to ejaculate is blocked, hence though testis are producing sperms they are not seen in the ejaculated semen. this condition is calle d obstructive azoospermia. in such men testis have a normal size, she and consistency and sperms can be retrieved with a needle from testis or epididimis, these sperms can fertilise eggs by ICSI. Some men may have genetic defects due to which sperm production at testis is arrested, others may have undesended testis, in these men also there is a chance of getting sperms by a microscopic biopsy called micro tese. So if your report is, azoospermia, cheer up you still have a chance to father your own genetic child!!!

Endometriosis is a disease where the cells of uterine lining called endometrial cells, get lodged and grow in response to ovarian hormones at various sites in the pelvis.posterior to uterus between rectum and vagina, below or inside the ovaries,and in the uterine muscles are common sites. As these cells respond to ovarian hormones they also shed during menses causing inflammation pai n and adhesions. Menstrual blood from these lesions collects in the ovaries forming thick tarry, blood filled cysts called chocolate cysts. Endometrial glands in uterine muscles cause localised enlargement of musculature, which is called adenomyosis, and gives rise to very severe menstrual pain. Menstrual pain, pain during intercourse and defecation along with infertility are chief symptoms of endometriosis. For treatment of infertility in women with endometriosis, IVF should be the first choice.before starting Ivf suppressing menses for 3months an

Important Update #5: Removing fibroids prior to IVF improves IVF success.

Fibroids are benign connective tissue tumours in the uterine muscles. They vary in size, number and location. Although they do not cause infertility unless inside the uterine cavity they have an impact on IVF success,American society of reproductive medicine recommends removal of fibroids in uterine muscles of size more than 2.5cms,and all fibroids inside the uterine cavity irrespective of size prior to IVF. So plan a laproscopic myomectomy way before you plan IVF!

Human embryo is covered with a hard protective shell called the zona pellucida. When the embryo is put back in the uterine cavity, it has to break this shell to implant and grow. Embryos produced in the IVF lab have a harder shell and needs more energy to break. If the zone of these embryo is opened, these embryos can easily implant in the uterine cavity. IVF specialists use a special infra red laser to open the zone of embryos from women with repeated failed implantation, advanced maternal age, slow growing embryos, frozen embryos and those with a thick zona. Majority of scientific studies favor laser assisted hatching of embryos to improve pregnancy rates in all the above conditions.
